Residential Habilitation

Residential Habilitation (ie. Res Hab) is a service available to HCBS participants. PHW provided the following documents with examples for when Res Hab may be appropriate for a participant and how to document the request in the PCSP Service plan and SRT. Though an SC does not make the Residential Schedule, an example is below as well, so that you can be familiar with how one would typically look for a participant.
