General Housing Resources
Community Connect is a PA Health & Wellness’ resource for local services. Just put in the participant’s zip code to find local housing resources where you can choose a program, view the next steps needed to start with it, or be connected to that programs’ website. Programs include:
- Help paying for housing/bills
- Foreclosure counseling
- Homebuyer education
- Maintenance and General Repairs
- Residential Housing (Long Term, short term, and safe housing)
- Independent Living
- Public Housing
- Assisted Living
- Nursing Home
- Temporary, Homeless, and Emergency Shelter
PA Housing Finance Agency also works to provide affordable homeownership and rental apartment options for senior adults, low and moderate income families, and people with special housing needs.
Philadelphia Housing Authority has their own Rental Assistance Programs and Rental Properties available in Philadelphia. Use the link or contact the following for assistance:

Homeless Assistance Programs through The Department of Human Services can connect the participant with organizations by county for:
- Case Management
- Designed to assist the participant in identifying their needs and the reasons why they became homeless or near homeless and provide them with the tools and skills that they need to prevent future homelessness.
- Rental Assistance
- If the participant is in danger of being evicted from their apartment or home, their county may be able to assist them with payments for rent, mortgage, security deposits and utilities
- Bridge Housing
- “The Bridge” that will move the participant from being homeless into permanent housing. Usually the next step after Emergency Shelter
- Emergency Shelter
- If the participant is currently homeless and has no permanent residence or is a victim of domestic violence

988 Suicide and Crisis Lifeline
The 988 Suicide and Crisis Lifeline is available for the participant and their family.
The 988 Lifeline connects the participant to mental health crisis support. They can call, text, or chat 988 for help 24/7. When they use 988, it is private and at no cost. The three-digit code is available to anyone having a mental health crisis.
They can use 988 for themselves or a loved one. A crisis counselor will be there to help. They will listen to the participant. They will work with them to understand how their problem is affecting them. They can share resources for additional help with:
Differences between 988 and 911:
Call 988 for:
- Thoughts of suicide
- Thoughts of hurting yourself or others
- Ongoing anxiety or depression
- Concerns about use of alcohol or drugs
Call 911 for:
- Someone’s life is in danger
- Overdose
- Emergency medical help
- Fear for your safety or someone else’s
Using the 988 Lifeline connects participants to someone right away. For help with a mental health crisis:
