  5. Home Mod/DME/SME Overview (1:30-2:15)
    • What are Home mods, DMEs, and SMEs
  6. End of Training Discussion (2:30-3:45)

    • Week 1 Performance Review meetings

*SC must have a caseload of at least 30 participants assigned by Friday afternoon


Training Week 2

  1. Daily Morning (9:00-9:30) and End of Day Team Meeting (4:00, 4:15, 4:30, 4:45)
  1. Monday:
    • Review of FP Dashboard, Client Scheduler spreadsheets*, make plan for the day (9:30-10:30)
      • Review caseload for previously scheduled visits
      • Review open tasks
      • Decide which participant need calls first (Monthly contacts, Tasks, visits scheduling, etc.)
    • Monthly Contact Training (10:45-11:30)
      • Review of Monthly Contact FP session
      • How to prepare for calls
    • Shadow 1-2 Monthly calls by LTS (11:30-1:00)
    • LTS shadows 1-2 calls by SC (1:30-3:00)
    • SC continues at least 2 successful calls independently
  1. Tuesday:
    • Review/make plan for the day (9:30-10:00)
    • Complete required calls/tasks (10:00-11:30)
      • Ideally at least 3 Monthly Contacts
    • Review of DSNP (11:30-12:30)
    • PHW Training meeting (1:30-2:30)
    • Shadow Visit (1:00-5:00)
      • Watch video, while completing and uploading InterRAI, HRA/DSNP, and FP Session 
    • If time permits, SC to complete more Monthly contacts/successful calls 
  1. Wednesday 
    • Individually review of shadow visit and completed documents (9:30-1:00)
    • SC to complete 3-5 successful calls
      • Monthly contacts/outbound calls/visit scheduled
  1. Thursday 
    • Review/make plan for the day (9:30-9:45)
    • Complete required calls/tasks (9:45-12:00)
      • Ideally at least 3 Monthly Contacts
    • Quarterly Contact Review (12:00-12:30)
    • Shadow Visit (12:30-5:00)
      • Complete Docusign, PCSP, and SRT.
        • Attach the InterRAI, and DSNP from Shadow visit #2
        • Attach Docusign certificate to FP session from shadow visit #2
    • If time permits, SC to complete monthly contacts/successful calls 
  1. Friday 
    • Review of shadow visit and completed documents (9:30-12:00) 
    • Incident Report Training (12:00-1:00)
    • SC to complete 3-5 Successful calls (Monthly Contact/Outbound calls/tasks)
    • Week 2 Performance Review Meetings (4:00-5:00)
      • Discuss expectations for Week 3 and on

 Post Training Expectations

Week 3
  1. Daily Morning (9:00) and End of Day Team Meeting (4:00, 4:15, 4:30, 4:45)
  2. 2 Annual or Change Event visits for the week 
  3. 2 Quarterly contacts for the week 
  4. 15 successful monthly calls for the week 
  5. All visits must be documented within 24 hours 
Week 4-6 
  1. Daily Morning (9:00) and End of Day Team Meeting (4:00, 4:15, 4:30, 4:45) 
  2. 3 Annual or Change Event visits a week 
  3. 3 Quarterly contacts a week 
  4. 20 successful monthly calls a week 
  5. All visits must be documented within 24 hours

*1 regular visit=2 Quarterly Contacts

Week 7 and on 
  1. Daily Morning (9:00) and End of Day Team Meeting (4:00, 4:15, 4:30, 4:45)
  2. 4 Annual or Change Event visits a week 
  3. 5 Quarterly Contacts a week (3 Friday and 2 more random during the week)
  4. 25 successful monthly calls a week 
  5. All visits must be documented within 24 hours 

*1 regular visit=3 Quarterly Contacts
