Training Week 1

Day 1 – Monday

  2. PHW and Orientation videos (10:30-12:30)

  4. Function Portal Introduction (2:00-4:00)
    • Logging into Test Portal
    • Review of Dashboard, Caseloads, and Client Information
      • Including Eligibility and Billing
    • Introduction to Tasks
  5. End of Day Meeting and Assignment (4:00-5:00)
    • SCS Team Meeting
    • EOD Assignment – Familiarizing with FP

Day 2 – Tuesday

  3. Function Portal Session Training (10:00-1:00)

Day 5 – Friday

  4. 1768 Form Training (1:30-2:15)
    • When it is needed

  5. End of Training Discussion (2:30-4:00)

Training Week 2

  1. Daily Morning (9:00-9:30) and End of Day Team Meeting (4:00)
  1. Monday:
    • Client Scheduler Spreadsheets and Monthly Contact Training (9:30-12:00)
      • Review of daily scheduler spreadsheet
      • Review of Monthly contact FP session
      • Shadow Monthly Contact by LTS
    • SC completes calls independently (12:00-5:00)
      • LTS shadows 1-2 calls by SC
      • SC completes to complete at least 2 calls per hour, including:
        • At least 3 Successful contacts and/or at least 6 Unsuccessful Outreach
  1. Tuesday:
    • Review of visit scheduling (9:30-10:30)
      • Review of daily scheduler spreadsheet
        • Identifying visits that need to be confirmed, rescheduled, or scheduled
      • How to schedule visits in FP
    • SC independently completes calls (10:30-12:00)
      • At least 3 successful contacts and/or at least 6 unsuccessful Outreaches
    • HRA-DSNP Training (12:00-12:30)
    • Shadow visit #2 (1:00-5:00)
      • Watch video while completing and uploading InterRAI, HRA/DSNP, and FP session
  1. Wednesday 
    • Critical Incident Training (9:30-11:30)
    • Review of Tasks (11:30-12:00)
    • SC to complete calls independently (12:00-5:00)
      • At least 5 successful calls and/or at least 3 Unsuccessful Outreaches per hour.
  1. Thursday 
    • PHW Services Review (9:30-10:30)
    • SC to complete calls independently (10:30-1:00)
      • At least 3 successful contacts and/or at least 3 Unsuccessful Outreaches per hour
    • Shadow visit #3 (1:00-5:00)
      • Complete Docusign, PCSP, and SRT
        • Attach InterRAI and DSNP from Shadow visit #2
        • Attach Docusign certificate to FP session from shadow visit #2
  1. Friday 
    • Visit Process training/review (9:30-11:00)
    • SC to complete calls independently (11:00-5:00)
      • At least 5 successful calls and/or at least 3 Unsuccessful Outreaches per hour
    • Week 2 Performance Review Meetings (4:00-5:00)
      • Discuss expectations for Week 3 and on

Week 3
  1. Daily Morning (9:00) and End of Day Team Meeting (4:00)
  2. 2 Annual or Change Event visits for the week
    • Schedule visits (Annual) at least 2 weeks before the due date in case of cancellation to remain in compliance
    • All visits complete or at UTL before due date
    • Complete pop up visit within 1 week after final UTL
  3. 15 successful monthly calls for the week 
  4. All visits must be documented within 24 hours 
  5. All tasks must be follow-up with on time
Week 4-6 
  1. Daily Morning (9:00) and End of Day Team Meeting (4:00) 
  2. 3 Annual or Change Event visits a week 
    • Schedule visits (Annual) at least 2 weeks before the due date in case of cancellation to remain in compliance
    • All visits complete or at UTL before due date
    • Complete pop up visit within 1 week after final UTL 
  3. 20 successful monthly calls a week 
  4. All visits must be documented within 24 hours
  5. All tasks must be followed-up with on time
Week 7 and on 
  1. Daily Morning (9:00) and End of Day Team Meeting (4:00)
  2. 4 Annual or Change Event visits a week 
    • Schedule visits (Annual) at least 2 weeks before the due date in case of cancellation to remain in compliance
    • All visits complete or at UTL before due date
    • Complete pop up visit within 1 week after final UTL
  3. 25 successful monthly calls a week 
  4. All visits must be documented within 24 hours 
  5. All tasks must be follow-up with on time
