  4. 1768 Form Training (12:30-1:15)
  5. Home Mod/DME/SME Overview (1:30-2:15)
  6. End of Training Discussion (2:30-3:45)
    • Review of topics
    • Expectations moving forward
    • Strategies and experiences as a coordinator
  7. Week 1 Training Performance Reviews (3:45-4:30)
  8. End of Day Team Meeting (4:30)

*SC must have a caseload of at least 30 participants assigned by Friday afternoon

Training Week 2

  1. Daily Morning (9:30) and End of Day Team Meeting (4:30) 
  1. Monday 
    • Shadow 1-2 Monthly calls by Lead Training Specialist (9:30-12:00)
    • Independently conduct 1-2 Successful calls (Monthly Contact/Outbound calls/Visit Scheduled session) monitored by Lead Training Specialist (12:00-3:00) 
      • About 30 minutes for each coordinator 
    • Continue conducting at least 2 successful calls independently (3:00-4:30) 
  1. Tuesday 
    • Shadow visit, complete and upload InterRAI, HRA/DSNP, and MCA 
    • If time permits, SC to complete monthly contacts/successful calls 
  1. Wednesday 
    • Review visit and completed documents with Lead Training Specialist (9:30-11:00) 
    • Incident Report Training (11:00-12:00) 
    • SC to complete 3-5 Successful calls (Monthly Contact/Outbound calls/Visit Scheduled session) (12:00-4:30) 
  1. Thursday 
    • Shadow visit, complete and upload Docusign and PCSP 
    • If time permits, SC to complete monthly contacts/successful calls 
  1. Friday 
    • Review visit/completed documents with Lead Training Specialist (9:30-11:00) 
    • Function Portal visit session training (11:00-12:00) 
    • Envolve Document Upload Training (12:00-12:30)
    • Review of Week 2 Training Checklist and Performance Review (1:00-3:00) 
      • About 30 min per coordinator 
    • SC to complete 3-5 Successful calls (Monthly Contact/Outbound calls/Visit Scheduled session) (1:00-4:30) 

 Post Training Expectations

Week 3
  1. Daily Morning (9:30) and End of Day Team Meeting (4:30) 
  2. 2 independent visits for the week 
  3. 2 Quarterly Low Risks for the week 
  4. 15 successful monthly calls for the week 
  5. All independent visit paperwork to be in compliance 
Week 4 
  1. Daily Morning (9:30) and End of Day Team Meeting (4:30) 
  2. 3 independent visits a week 
  3. 3 Quarterly Low Risks a week 
  4. 20 successful monthly calls a week 
  5. All independent visit paperwork to be in compliance 

*1 regular visit=2 QV Low Risks

Week 5 and on 
  1. Daily Morning (9:30) and End of Day Team Meeting (4:30) 
  2. 5 independent visits a week 
  3. 5 Quarterly Low Risks a week 
  4. 25 successful monthly calls a week 
  5. All independent visit paperwork to be in compliance 

*1 regular visit=3 QV Low Risks
